In order to find out your rising sign, you need your exact birth time, birth place, and birth date. The rising sign is the sign coming over the eastern horizon at birth, and it changes about every 1-1/2-3 hours so the time is critical.
In order to find out your Moon sign, you also need the same info, but if your birth time is only approximate you can usually still get the Moon sign. The Moon changes signs every 2-1/2 days.
