Hey guys

With all the recent talk about our prostate both here in our tribe and in the press, it appears not many of us know what it actually does ...so I thought you might like to get an overview of what this tiny organ actually does and this article is a good introduction. BTW I have granted permission to reprint this article here, from the Sydney Morning Herald.

Back when I was practising medicine, I was a regular at Sydney Long Bay jail, teaching the prisoners some basic anatomy and physiology relating to sexual matters. At first, I was a bit taken aback by how little the guys knew. But then I remembered how little I knew about my own body before I studied medicine.

That said, you’d still think that an organ that gets as much publicity as the prostate does these days would be familiar to the half of the population that owns one.

In fact, according to a 2008 survey of 503 men aged 40 to 80 in WA, 80% didn’t have a clue what the gland did. And this was despite the fact that 75%of them had had a previous prostate related examination, and that about 50% of them had had dealings with prostate cancer (either through personal experience or when a friend had it).

So here are some basic facts. The prostate gland was first described in 1563 and its cancer identified in 1853. In the US and UK, cancer of the prostate is the second most common cause of cancer deaths in men , after lung cancer.

In Australian men, it’s the second most common cancer after skin cancer and is the No 2 cause of cancer deaths.

Most prostate cancer sufferers do not have any symptoms, do not get any treatment, and end up dying from other causes. This is because cancer of the prostate can grow very slowly, and most sufferers are more that 50 years of age.

Indeed, in studies of men who died from other causes, autopsies show that cancer of the prostate was present in 30% of men in their 50’s and in 80% of men in their 70’s.

The prostate gland is about 3cm long, and weighs about 20 grams. It is usually said to be the size of a walnut or chestnut. It’s located deep in the pelvis directly under the urinary bladder, and immediately in front of the rectum. The urethra, after leaving the bladder on its way to the outside, passes through the middle of the prostate gland. So as the prostate gland enlarges with age, it can both constrict the urethra (interfering with urination and sexual ejaculation) and bulge upwards into the bladder (giving a false sense of bladder fullness).

The tissue of the prostate gland is about 5% non-glandular (with fibro-muscular components and 95% glandular (from which cancers of the prostate arise). In the glandular part, there are some 30 to 50 little glands secrete various fluids (collectively known as prostatic fluid) into pipes the eventually lead to the urethra. Prostatic fluid is a clear, slightly alkaline liquid with a rather characteristic smell. The alkalinity is thought to improve the survival rate of the sperm in the vagina.

Prostatic fluid makes up about 10 to 30 % of the volume of the semen, with about 10% coming from the vas deferens, and the remainder coming from the seminal vesicles.

The prostate gland makes prostatic fluid but also has a muscular function. During ejaculation, it provides exquisitely timed waves of muscular contraction to help propel semen to its final destination.

Now that the prostate is increasingly a media darling men might become increasingly aware of its purpose. [end of reprint]

    I found this on Wikipedia under Prostate Function:

    "The function of the prostate is to store and secrete a slightly alkaline (pH 7.29) fluid, milky or white in appearance, that usually constitutes 25-30% of the volume of the semen along with spermatozoa and seminal vesicle fluid. The alkalinity of semen helps neutralize the acidity of the vaginal tract, prolonging the lifespan of sperm. The alkalinization of semen is primarily accomplished through secretion from the seminal vesicles. The prostatic fluid is expelled in the first ejaculate fractions together with most of the spermatozoa. In comparison with the few spermatozoa expelled together with mainly seminal vesicular fluid those expelled in prostatic fluid have better motility, longer survival and better protection of the genetic material (DNA).

    The prostate also contains some smooth muscles that help expel semen during ejaculation."

    Very similar to what you posted.....

    I believe that male health (and especially sexual health) is a poorly understood and closeted topic so it's great to have a forum, such as this one, for us guys to discuss things like the prostate openly.

      In discussing this topic with a great mate of mine on the phone the other day....a GP told him that the reason this is a older mans complaint is due to the fact that the sexual desire is reduced as we age....and as the cancer cells feed on built up semen, hence the beginning of the complaint......soooooo ...

      in order to prevent this, we all should be wanking as much as we can.....at least a few times a week.....

      With that in mind I have started and am already up to December, and that is just from yesterday....ROFL.....

        bloody hell...everyone has a head start (no pun intended) so I am typing this slowly with one hand...the other one is busy.
        Seriously though, I had a call last night from a close long time mate (just turned 65) and he was diagnosed yesterday with an aggressive form of prostate cancer.
        I said that I had the PSA blood test a couple of weeks ago which came back normal (I have had that done every year for the past 10 years) and also the physical exam by the doctor..which has also been done every year....was okay as well.
        What staggered me was that he said his last prostate test was 3 years ago even though he visits his doctor every year for check ups etc. His doctor is a woman...which should not make any difference...and no prostate PSA or digit exam was done or even suggested.
        I must be naive as I thought it was something automatically done by every doctor....or am I just lucky that I go to a male doctor who is concerned and up to date on male health.
        So..all you men out there......insist on having it done. What there is to fear having it done I do not understand and no, it will not go away if you do not know about it.
        God, that takes a long time to type with one finger.........still busy here so concentration is needed.

        having recently been diagnosed with this cancer - i ve dont some fairly extensive study, and gained heaps of information on the prostate and its cancer.
        Reality is - the cancer does not feed on "semen" it feeds on testosterone, which is produced by Androgen. In my treatment, I taking a course of hormones (andracur) who's job it is to reduce the androgens production of testosterone ( on which the cancer feeds.) By reducing the the testosterone, the 'feed bucket" for the cancer is smaller - and makes it then easier for the medicos to target the remaining cancer cells with a deep radio therapy beam --- for me that should happen in around january. Hopefully it will kill all the cancer cells within my prostate ---a further course of hormones after the radio therapy will then be started to "clean up' any left over cancer cells

          You have spoilt all of my fun now Allan ....fancy telling me it's to do with a reduction on testosterone and not semen.....do you mean to say I have been wanking day and night for nothing????

          Anyway i enjoy it so ......

          BTW Ron, it has been proven that people who multi-task have a less that satisfactory outcome on either task.....and it's been found you would be better to concentrate on your typing with both hands, or apply both hands to what other task you were doing and forget the typing....*wink*

            Jim I watched this programme = and it was good --- my only real criticism is the suggestion that we shouldnt get tested.

            To me that is a silly, ill informed and dangerous notion - and i've fired of a note to the ABC advising of my view.

            Had it not been for tests I wouldnt know that I have the cancer --- and while it MAY not manifest itself in me for years to come, who is it that decides the date on which the cancer spreads beyond the prostate and into the general body?? is it me? the medicos?? or the Cancer itself?? Im thinking the cancer makes that decision, and a test will help US to take control of the decision making.

            to discourage guys friom having tests that will reveal a health problem is to my mind bordering on criminal - but then I know what the abc is like, I worked with it for 20 years --- and i've yet to gain prove that they are at all responsible ...

            (remember the breast cancer cluster at their Toowong office? The then News Director on Fiona Crawford went on television and made the statement "I dont know what they're complaining about" ... with That attitude toward their staff, why would their attitude to their viewers be any different?/

          just a bit of a correction to the suggestion that the cancer cells "feed on" built up semen ------- not quiet right,.

          After regular discussinos with my GP, my Urologist, my Oncologist and uncle tom cobly and all - and checking the veritable library of information put out by the cancer council, the prostate cancer support group, the Wesley Hospital prostate cancer people you name it ---- the notion that Cancer feeds on built up semen is a bit of a myth.

          However -what the cancer cells DO feed on is Testosterone.

          and it is for this very reason, that my medicos have chosen this course of treatment for MY prostate cancer.

          I am currently on a course of Androcur tablets - a female hormone, who's job it is to "stop" the production of testosterone ( typical huh? another way women get to control men! LOL)

          by 'stopping" the testosterone production the food supply for the cancer cells is taken away, and subsequently many ( though not all) will die

          It will reduce the "area" within the prostate that is cancer- affected --- providing one very clear target for the radiation treatment, ( which Im told will probably actually start in about March 2010,)

          Thjats the short version.

          As for other suggestions that increased sexual activity will stop the cancer --- thats fine, but dont place too much trust in that theory either.

          The greater your sexual activity, the more testosterone you will produce, and the more 'food' y ou are providing the cancer ..


          A strong theory now being discussed is that all cancers are caused by a virus .... they just dont know what virus or how it causes the disase ....
