I've volunteered to be the blinder for the request process, and my immediate goal is to have a web page that a requestee fills out which generates an email to me for blinding.
I will start with the paper based form, and should have a proto-type by our meeting on the tenth.
While I'm working on it, I'd be happy to hear any/everything people are thinking about what the form whould include.
